1) Upon termination of your member account, you may retain the accrued benefits (including Mandatory and vested Voluntary Contributions) in a Preserved Account with Manulife under your own name. In fact, if we do not receive your instruction within 90 days after your member account termination, your accrued benefits will be transferred to a Preserved Account automatically. On the other hand, you may transfer the accrued benefits to your contribution account with the new employer or the member account in another scheme by submitting the "Election Form for Transfer of Accrued Benefits" to the transferee trustee.
2) You may elect to withdraw the vested Voluntary Contributions from your MPF account upon termination of employment. Kindly complete and return the "Election Form for Transfer of Accrued Benefits" by fax to (852) 12345678. We will proceed your request accordingly. The mentioned form can be faxed or sent to you upon request.
